Apply for R.A.R.E. Grant Funds

The R.A.R.E. Fund provides financial support to Providers offering services at a discount or cost free to individuals with chronic invisible illnesses, such as traumatic brain injury, PTSD, chronic pain, or autoimmune disorders, which affect daily life without visible signs.

To qualify, individuals must have had a past trauma, such as domestic violence, sexual assault, human trafficking, or traumatic accidents.

Services provided must directly support the management of the patient’s condition and improve their quality of life.

How can I join the R.A.R.E. Support Network as a Provider?

To become a provider in the R.A.R.E. Support Network, simply apply through our online intake process. Once your application is reviewed and approved, you’ll be listed in our provider directory, allowing patients to find and contact you directly. Additionally, we’ll schedule an onboarding call to walk you through the process and ensure a smooth transition.

What are the reporting requirements?

Reporting Requirements for R.A.R.E. Support Network

Electronic Reports

  1. At the Time of Service

    • Providers must submit an electronic report immediately upon delivering a service funded or discounted through the R.A.R.E. Fund.

  2. End-of-Month Reporting

    • A consolidated electronic report is required at the close of each month. This report ensures comprehensive documentation of all services rendered within the reporting period.
